Siefert Elementary understands the connection between family support and student success; therefore, we are committed to involving families in their child’s education. A strong support network between home and school helps a child to achieve their full potential in life.
Families are encouraged to take part in decision-making at Siefert School by joining the School Governance Council and participating in surveys. A Family Center is available to welcome and support parental involvement. By volunteering in classrooms or chaperoning field trips, parents can take an active role in educating students at Siefert School.
Student Forms
The forms below can be downloaded, printed or filled out online. If you want to fill them out online and email them, you will need to download software to your device first:

The forms are set to be emailed to the office if you click the Submit button or you may print the forms and send them to school with your child.
Family Required Forms Packet
Please start with your oldest child at Siefert first. In the sibling section list the other family at Siefert. If all the children in your home attend Siefert and they all have the exact same information, you only have to complete one contact card. If any of your children different information, you have to complete a separate card for that child. You may safe and print the forms for your records. Click submit and they can be emailed to Siefert.

- Covid-19 Testing Consent – MPS will be randomly testing staff and students for Covid-19. Students whose parents do not consent to testing or do not return the form by the deadline will not be allowed be in school. They will have to stay home and attend school virtually. Free Smoke Detectors
For any family living in a single or multiple family house or condominium in the City of Milwaukee. If in need of a smoke alarm, an adult can call, leave their name, address and phone number. We will call back to schedule an installation appointment and install a 10 years lithium smoke alarm for FREE!!! People living in apartment buildings need to call the Department of Neighborhood Services if they need a smoke alarm 414-286-2268.
